HLTH 2024 Group Block Request Form

Room block requests are assigned on a first-come, first-served basis, and based on availability. Room type requests are not guaranteed.

Connections Housing is the only housing company authorized by HLTH to provide housing services to HLTH 2024 participants. Please do not conduct business with any other travel or housing company claiming to represent this event. Should you happen to receive communication from any outside company, please forward their contact information to Connections Housing via email at housing@hlth.com

GROUP HOUSING TERMS & DEADLINES:

Group Housing forms will need to be signed with the understanding that all reservations made through group housing blocks are considered fully pre-paid and non-refundable* after August 15, 2024.

Payment Method/s: Group Housing reservations will be invoiced for FULL PREPAYMENT FOR ALL ROOMS via ACH/Bank Wires (subject to bank fees) or credit card (subject to processing fees). Group reservations are secured with fully pre-paid, non-refundable for room, resort fees, and current applicable taxes for each reservation prior to hotel arrival. Incidentals due at check-in with a valid credit card. 

*PLEASE NOTE: NO ADJUSTMENTS WILL BE MADE TO YOUR INVOICE WHEN YOU SHORTEN STAYS AND MAKE ROOM CANCELLATIONS AFTER AUGUST 15, 2024, i.e. Your group submits 24 room nights on August 5, 2024: If you cancel a room reservation with 3 nights after August 5, your total due will remain at 24 room nights regardless of dropping reservation(s). This is your financial responsibility when you submit a signed Payment Agreement Form and you will be charged post-event.

ROOM BLOCK REQUESTS AND ROOMING LIST DEADLINE:

Deadline for room block submissions is ON Monday, August 15, 2024. The Housing team will need the individual names, bed type requests and arrival/departure dates for each room in your hotel block by this date. After this date, any room that is not assigned a name will be released back into general inventory for others to book.

Group remains responsible to satisfy any obligations of Group’s attendees to Hotel. Group must supply Hotel with a valid credit card which may be used in the event any funds remain due Hotel. If the provided credit card is declined and Group does not provide full payment promptly via another means, Group hereby authorizes HLTH to make payment to the Hotel on Group’s behalf and to collect the amount of the shortfall from company’s depository Checking Account in accordance with the terms of company’s Agreement with Group.
